Job Summary

Medical Services at CU Boulder encourages applications for a Registered Nurse Case Management and Resource Coordinator! This position is a critical role within the Health & Wellness - Medical Services Division, aimed at enhancing the well-being of our patients through effective case management and resource coordination. The role will be a liaison between patients using Medical Services who are navigating health-related challenges and the various resources available within the academic institution, and within the local community. The role works closely with healthcare providers, counseling services, and administrative departments to ensure patients receive the support needed to thrive academically and personally. This role requires compassion and a solid understanding of health services, excellent social skills, and an ability to adapt to a dynamic environment. Responsibilities include developing personalized case management plans, tracking patient progress, and advocating for patients' needs, all while maintaining confidentiality and compliance with relevant regulations. What Your Key Responsibilities Will Be

Case Management and Resource Coordinator
  • Develop and implement comprehensive case management plans tailored to individual patient needs through collaboration with Wardenburg Primary Care and Sexual and Reproductive Health Providers.
  • Serve as the primary point of contact for patients receiving medical care and seeking assistance with complex medical and health-related issues.
  • Coordinate with patients, families, healthcare providers, academic staff, and external agencies to facilitate access to resources and services.
  • Monitor and evaluate patient progress, adjusting case management strategies as necessary.
  • Advocate for patients to ensure they receive adequate support and resources.
  • Maintain accurate documentation and records in accordance with privacy regulations and institutional policies.
  • Facilitate medication refill requests and when necessary, initiate, submit, and process medication prior authorizations.
  • Use standing orders to call-in medications for specific diagnoses including strep, flu, and other diagnoses.
  • Assist referrals office with coordination and follow-up of referrals for patients that have been referred outside of the health clinic.

What You Should Know

This is an in-person position with an anticipated work schedule of 4 days a week during the 80% time from August through May, and 2-3 days per week during the 50% time of June and July. Working days will be between Monday - Friday, and this role is not expected to work weekends, evenings, or University recognized holidays.

What We Can Offer

The annual salary range for this 80/50 position is $52,096.50 - $65,139.75.

What We Require

  • Bachelor's degree in nursing.
  • Two years of experience as a Registered Nurse.
  • Active, unencumbered Registered Nurse license in the state of Colorado by the date of hire.
  • Active Basic Life Support certificate by date of hire.

What You Will Need

  • Strong understanding of healthcare systems, university systems, resources, and services available to patients.
  • Excellent communication and social skills, with a focus on emotional intelligence and active listening.
  • Critical thinking and prioritization to determine the best care plan for each patient.
  • Flexibility to change care plans to drive the best outcomes for patients.
  • Ability to work collaboratively with varying populations and partners.
  • Proficient in using technology and electronic medical records for documentation and communication purposes.
  • Great organizational skills and the ability to manage multiple cases and priorities effectively.

What We Would Like You to Have

  • Two years of experience in ER, Urgent Care, Student Health, or Primary Care.
  • Proven experience in nursing and/or case management, especially within a higher education or health services context.
  • Familiarity with Boulder County and Denver Metro Resources.
